How to provide notarized copies of documents to Graduate Admissions

Instructions for providing Notarized copies of documents to the Office of Graduate Studies at Georgia Tech.
  1. Print the Notary form attached below
  2. Take the form and your official documents to the notary.
  3. Have the notary make copies of your documents and complete the form.
  4. Mail the completed form and notarized copies of the documents to the Graduate Studies office at the address listed below.
